  1. You are responsible for doing an investigation into a reportable (to Department Of Labour) incident that occurred at your workplace.\n
      \n
    1. Write in detail what happened during this serious incident (1 – A4 Page).<\/li>\n
    2. Describe what losses may be suffered by your company and employees involved in terms of: Financial; Social; Emotional; Environmental losses (1-2 A4 Pages approx. 500 words)<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n

      1. Explain who will be involved in this investigation. List respective investigation team members with their individual duties and responsibilities during this investigation.<\/li>\n<\/ol>\n

        Draw a timeline of the investigation processes.  Describe the procedures to be followed during each event on your timeline.  Give a detailed summary of actions by your investigation team and explain why these actions are important to follow during your investigation.<\/p>\n

        1. It is 10:00 am Monday morning. You are busy with a meeting at work when you are called by the receptionist, – There has been a fatality at the reception area OR boom gate entrance of your company.<\/li>\n<\/ol>\n

          List from this moment 10:08 am on, what you will do for the next 24hours to demonstrate your skills as a good incident investigator REMEMBER to:<\/p>\n

          4.1 Outline a simple process to activate Emergency Services to limit further injury to people and damage to the site.<\/p>\n

          4.2 Describe specific emergency responses to an incident scene with regard to restricting injuries and to   containing damage.<\/p>\n

          Specific Outcome 3:<\/u><\/strong>       Investigate the incident and describe the review\/follow up process.<\/strong><\/p>\n

          Specific Outcome 3 answers should consist half an A4 page each<\/u><\/em><\/p>\n
